  • Role and Responsibilities 

    •  Initial in-home assessment, development of the plan of care as applicable, and regularly supervises the needs of the client every 90 days. 

    • Manage in-home aides to ensure the level of care is being rendered according to the plan of care. 

    •  Observe and report changes in the patient’s physical or emotional condition. 

    • Initiate preventative procedures as appropriate for the patient’s care and safety and effecting changes in the environment to ensure patient safety and security  

    •   Coordinate services for patients as needed to other agencies and health care providers as appropriate. 

    •  Provide supervision of caregiver services and prepare written instructions for care provided by aides  

    • Maintain confidentiality of patient and agency matters  

    •  Submit required documentation in a timely manner  

    • Report observed or suspected abuse pursuant to mandated requirements.  

    • Assist the Administrator in decision-making and policy formation as it pertains to the Home Care Services.   

    • Maintain accurate time records 

    •  In addition to scheduled hours, participating in weekend/holiday call schedule, as needed. 

    • Observe the agency’s attendance requirements and dress code.  

    • Conserve agency resources and adequately maintain agency property, supplies, and equipment.  

    • Adhere to all agency policies, including but not limited to, infection control and safety, education, reporting, and practice implementation. 

    Qualifications and Education Requirements

    •Graduate of an accredited school of nursing  

    • Valid and current RN license in the state of North Carolina  

    •   CPR & 1st aid certified, good standings on a nurse registry  

    •  1-3 years of nursing experience (home care experience, preferred)  

    • Supervisory skills, a plus  

    •  Excellent communication skills, flexible, and able to interact positively with the rest of the staff.  • Strong sense of initiative and urgency in addressing unexpected patient crises  

    • Self-directed, able to work without the presence of immediate supervisor.  

    •  Commitment to regulatory compliance and to submitting all documentation in a timely fashion  

    • Demonstrated knowledge regarding exposure to body fluids and chemicals, as well as to some hazardous equipment  

    • Must have the ability to: 

    • See and hear adequately to respond to auditory and visual requests. 

    • Pass a background screen. 

    •  Travel as required and have a valid driver’s license and automobile insurance. 

    •  Evidence of adequate health status to perform duties 

    • Possess excellent computer skills, and complete required forms, and documents promptly to ensure compliance.  

    •      Physical abilities to adequately perform described duties

  • Adhere to the plan of care which involves touching the patient in order to implement the patient's plan of care i.e. home management tasks, personal care tasks, or supervision of the client's activities to enable the individual, their family, or both to remain and function at home. 

     Qualifications & Responsibilities:

    -High school graduation or GED required. 

    -18 years or older. 

    -Must be listed and in good standings on the NC Health Care Personnel registry. 

    -Ability to read, write, follow instructions as appropriate, for understanding the needs of populations served, personal care tasks, home management tasks, basic meal preparation, provision of transportation services, housekeeping, home safety, handling emergencies in the home and infection control. 

    -Evidence of sympathetic attitude toward care elderly. 

    -Evidence of maturity and ability to deal effectively with job demands -Good verbal communications skills required. 

    -Attends (8) hours of caregiver oriented in services per year. 

    -Shall have a criminal history check conducted prior to being offered work with this agency. 

    -Ability to work closely supervised to assure competence in providing client care. 

    COMPETENCIES: 

    -In-Home Aides must be trained and competent to perform client care tasks or activities to which they are assigned. Such individuals shall perform delegated activities under the supervision of a Registered Nurse. The aide must demonstrate competency with a successful skills check off by a RN. Description text goes here.

  • Assure quality and safe delivery of companion, sitter, and respite services. 

    Qualifications and Responsibilities

    Caregiver must provide home management tasks, shopping, meal preparation, transportation, companion services, socialization, medication reminders, and other services to assist the client in the home excluding “hands-on” tasks.  

    -Relieving the family of duties which are planned in advance and or in an emergency. 

    -High school graduation or GED required. 

    -18 years or older. 

    -Ability to read, write, follow instructions as appropriate, for understanding the needs of populations served, basic meal preparation, provision of transportation services, housekeeping, home safety, handling emergencies in the home and infection control. -Evidence of sympathetic attitude toward care elderly. 

    -Evidence of maturity and ability to deal effectively with job demands -Good verbal communications skills required. 

    -Attends (4) hours of companion, sitter, respite oriented in services per year. 

    -Shall have a criminal history check conducted prior to being offered work with this agency. 

    -Ability to work closely supervised to assure competence in providing client care. 

    Competencies

    - Companion and sitters must be competent to perform client care tasks or activities to which they are assigned. Such individuals shall perform delegated activities under the supervision of a supervisor designated by agency policy for the services assigned.
